AUTHORIZATION FOR RELEASE OF INFORMATION
I acknowledge that the purpose of the University of Notre Dame’s Talent Search Program is to prepare me to successfully graduate from high school and succeed in a postsecondary

institution of higher education. As a federally funded program, Talent Search must abide by the U.S. Department of Education’s rules and regulations in regard to tracking former students

(Authorization –  U.S. Department of Education: 34 CFR Part 645.32 (b) (4) and(b) (5)*). As such, I understand Talent Search’s obligation in obtaining secondary school records and academic

documentation six years following the end of high school graduation and enrollment into a two- or four-year postsecondary institution.

Therefore, I hereby authorize for all high school and postsecondary documentation to be released to the University of Notre Dame Talent Search staff members.

I am signing this Authorization Form with the knowledge that this document is valid for the next twelve (12) years for the purpose of tracking academic information.


*Title 34—Education § 645.32 (b) (4) The extent to which participants who complete the project, or were scheduled to complete the project, undertake programs of postsecondary education; and (5) The extent to which participants who complete the project, or were scheduled to complete the project, succeed in education beyond high school, including the extent to which they graduate from postsecondary education programs.
